~ Please arrive 15 min. before your appointment to fill out the new patient paperwork (initial visit only) or you may click here: New Patient Forms to download, print, and fill out the forms. You will need to bring the completed forms with you to your first appointment.
~We are not an insurance provider. We cannot accept insurance as a form of payment or co-payment. Payment is expected at the time of service. We can provide you with an itemized receipt for you to submit along with your claim form to your insurance carrier directly as a complimentary service for a more prompt reimbursement. We are not accepting Worker's Comp or No-Fault patients at this time.

~Gowns &/or shorts are provided for your appointment.
If you are uncomfortable with wearing a gown, please wear loose comfortable clothing.
